Mark the level of access you are requesting:
"College", "Department" and/or "Individual". |

If you request College and/or Department, please list the College(s)
and/or Department(s) and complete Section A. |

If you request access to individual SET reports, please complete
Section B. |

Obtain an Authorizing Signature. |

4. |
