Certificates of Insurance
A certificate of
insurance provides proof that required insurance
has been obtained. If the other party does not
request a certificate (either verbally or in
writing), do not offer to send them a Certificate.
If you are verbally asked to provide a
Certificate, please ask them to send you a written
request (fax or email is acceptable). Certificates
should only be furnished when requested by another
party IN WRITING.
NOTE: Please be aware that even a
“simple” agreement may have indemnity language
that is hard to identify but will create an
obligation on the part of the University. Any
contract containing an indemnity (“hold harmless”)
clause must be signed by the Vice President for
Finance and Administration. Failure to obtain the
required signatures may result in personal
liability for the unauthorized signer. Please
consult with us prior to signing contracts.
For use of the forms below, please
refer to
Contracts.
Additional information may be
required, based on the specifics of your request.
