After reading the Survival Guide, please download and fill-out the forms below that pertain to you. Submit these forms to the UI Study Abroad Office prior to the deadlines in order to avoid the $100 late forms penalty fee:

Deadlines:	April 15 - for summer, fall, and academic year participants November 1 - for spring participants

UI Students:

Form #3: UI Information Release (pdf)
Form #4: Advising and Credit Evaluation Agreement* (pdf)
Form #5: Assumption of Risk & Release (pdf)

(*not required for UI Faculty-Led Programs)

Non-UI Students:
If you are studying abroad through the UI but not originally a UI student, you are considered a "Non-UI Student". This also pertains to students who have applied to the UI as degree-seeking students for financial aid purposes.
