Instructions for
University of Idaho Consent for Release of Information Form
Reason for release: If you have entrusted your parents,
guardians, significant other, or someone else to handle anything for
you while you are abroad (emergencies, finances, registration,
etc.), no one at the University of Idaho will be able to give them
any information about you or these topics if you have not signed
this form. This is due to FERPA (Family Educational Rights and
Privacy Act).
Please return this form (marking
your consent or non-consent) to the UI
Study Abroad Office at the information below with your other UI
Study Abroad Mandatory Forms.
Do not
send the form to the Registrar’s Office address that is listed on
the form.
